2008 Wellness Program We are now getting ready for our 4th year of our Equine Wellness Program. We are excited to begin another year. We have had great success in prior years. We would like to thank everyone that has participated and look forward to seeing you again in 2008. Our mission is to provide high quality veterinary services and continuing education to our clients. Through this wellness program, we are offering a way to ensure that your horses are protected through thorough examinations, scheduled vaccinations and de-wormings. We can also advise you concerning your horses health , nutrition and dental care. By enrolling now, our office will schedule your horse for all necessary exams, vaccines, Coggins test, and de-worming for the next calendar year. This ensures that your horse receives everything needed when it is due. As a bonus for enrolling in the program, you will receive a 10% discount for 1 horse, 12% for 2 horses, and 15% for 3 or more horses. All enrollments are prepaid and must be received by . January 2008. Wellness Program Terms: The hospital staff will contact you to schedule appointments twice per year. Should your horse(s) need: a. Dental alignment and/or float b. Sheath cleaning
to these two services when completed within the same year. Should your horse(s) be sold to a new owner, or should your horse pass away, you will be refunded for the remaining services that were not performed.
